Player Injuries

Lacombe Minor Hockey Association has Return to Play Protocol for injured players.

If a player is injured during a team sanctioned event then a Hockey Alberta injury form must be filled out and returned to your coach.
The form can be found on the Hockey Alberta Website https://www.hockeyalberta.ca/members/insurance/insurance-forms/

Once a player is ready to return to play, a doctors note approving the return must be given to the coach prior to participation
